If we go by statistics, Qantas of Australia is the safest in the world, since they have not had a fatality in all the years that they have been incorporated under that name.

Also, spending aside, the more planes an airline has, the more likely it is to be involved in an accident and to lose lives, so from a numbers standpoint, American Airlines (the world's largest airline), has a higher probability of being involved in an accident with a loss of life.

My best guess would be NorthWest airlines or another one of their affiliates in the Sky Club program. Those seem to be of the higest quality airline travel in the world. Generaly, the more expensive the ticket, the higher the standards are of the airline itself and quality of the aircraft and skill of the pilots.
