Travel Tips
Go to firstgov.gov and then click on your state. Then search passports. You'll get the application and do it on line. Just have to get two passport photos on the outside to mail in with your check. A Walgreens or CVS shoul take the photos.

i went down to this gigantic post office awhile ago and got mine but here is a better link...http://travel.state.gov/passport/passpor...
The Department of State will be able to help you out. Responsibility for their issuance lies with the Passport Services of the Bureau of Consular Affairs within the Department of State, which has numerous acceptance agencies and agents. You may also need a visa from the relevant country depending on which country you will be visiting.

Regarding travelling in a wheelchair, you will need to do more research. Is there wheelchair suitable transport available? Are the places you want to go wheelchair accessible? How convenient is your accommodation and is it wheelchair accessible? If travelling alone, are there any nursing or caring services available?
