Up through 12/30/2006 a passport will not be required to re-enter the US from Canada by air. A birth certificate and a government-issued photo ID can be used in lieu of a passport. If you are a married woman who has changed her name, you will need your marriage license as proof of the name change.

12/31/2006 and later, a US passport will be required to re-enter the US.

Canada has no requirements beyond proving US citizenship - similar to the current process for re-entering the US.

No. I recently traveled to Canada from the US, and did not have a passport, only photo ID. While it's not absolutely necessary, I would HIGHLY recommend having your birth certificate, in addition to the required photo ID. And as Clare_Here pointed out, you should also have naturalization papers if you weren't born in the US.

Citizens of the United States do not need a passport or visa to enter Canada, unless they are arriving in Canada from somewhere other than the USA. They do need to have on them proof of citizenship plus photo identification, although this is not often asked for. At this time you do not. You just need a valid, government issued ID with picture (like a driver's license) and your birth certificate or your naturalization certificate.
